Clinical Model & Staffing

Your patients get evaluated, prescribed for, dispensed to, and monitored under one roof. Here's how that's actually staffed and structured so it holds up.

No referring out

Today, a patient who wants hormone therapy or metabolic care gets sent to a telehealth company or a specialist across town, and you never see that business, or that patient's full picture, again. Under HARP Wellness, evaluation, prescribing, dispensing, and ongoing monitoring all happen inside your pharmacy. The member never leaves the relationship they already trust.

How the clinical team is structured

Pharmacy law requires a specific structure here, and we build to it rather than around it. A physician-owned medical practice, affiliated with HARP, holds the prescribing authority and clinical decision-making. It contracts with HARP's management company for support services, but the clinical decisions always sit with licensed clinicians, never with a management company. That separation is required by law in most states, and we design for it from day one rather than retrofitting later.

The prescriber
A licensed physician or nurse practitioner evaluates the patient and initiates treatment: hormones, metabolic care, and other services that require a prescriber. In states with full-practice-authority for nurse practitioners, this role is more affordable to staff, which is one reason we're starting in the Mountain West.
Your pharmacist
Under a collaborative practice agreement, your pharmacists carry the ongoing management (reviewing regimens, adjusting dosing, and monitoring) under a protocol delegated by the prescriber. This is the same trusted relationship your patients already have, extended into an area they couldn't access from you before.
Dietitian / wellness advisor
Handles nutrition consults and coaching included in higher membership tiers, working from the same biomarker data as the pharmacist and prescriber.
What a pharmacist cannot do
Pharmacists cannot prescribe controlled substances like testosterone under current law, no matter the protocol. That's why a licensed prescriber is part of the model, but it's our prescriber, on site or closely affiliated, not a third-party referral that breaks the relationship.

Why everyone is salaried

No clinician on the team is paid on commission for product sold, and that policy is published, not implied. It's the simplest possible answer to the fair question every patient and every referring physician eventually asks: is this recommendation about my health or about a sale. Salaried clinicians protect the trust that makes the whole model work, and protect you, since your name and your pharmacist's reputation are what patients are actually trusting.
